You
can quickly check status of claims by accessing our Interactive
Voice Response System (IVR). In addition to the date of service,
your patient’s ID number will be needed. This is located
on your ID card and begins with three letters. Please call
(503) 279-9943 or (800) 824-9789 and select option 1.
Plan participants can also contact a Benefit Specialist by
calling 503-279-9943 (local) or 1-800-824-9789.

How to Request an ID Card
ID cards are issued to you and your spouse upon enrollment.
If you wish to obtain additional copies or a replacement,
